Prior to your appointment, we ask that you fill out the patient forms attached at left as Words documents, so we can accelerate your check-in process. Please download, print and complete the forms and bring them with you to your appointment.
If you cannot download our patient forms before you visit, we recommend arriving to your appointment 15-30 minutes early so that you can have ample time to fill out the appropriate forms
Woodstock Smiles Insurance Policy
Our team of experienced front office professionals remain committed to helping our patients receive the maximum benefits available under their current insurance plan. We happily file all insurance claims on our patients behalf, and will work with your provider to ensure you pay the lowest amount possible.
We ask that all of our patients with insurance handle a few responsibilities that include:
- Understand your insurance benefits. If you need assistance in determine what benefits you have available, we will happily help provide you answers.
- We please ask that you have all relevant insurance information available during your first appointment. This information includes: name of employer, name of insurance provider, group number, contact number, social security number, and alternate employee ID, if provided.
- On the day you receive treatment, we please ask that you pay any deductible and your patient portion.
- If a balance remains following the filing of your insurance claim, we ask that you please pay the outstanding amount promptly.
